ORDERING DIABETIC TESTING SUPPLIES

Cash Customers:

If you are a cash customer and want to purchase any of our diabetic testing supplies, simply call the toll-free number above. We can take care of your order in just a few minutes. We accept Mastercard, Discover, and Visa for phone orders.

Medicare and/or Medicaid:

If you're on Medicare or Medicaid, we need to know some information about you before we can process your first order. Please take a moment and fill out our New Patient Profile form.

Then, we need your written approval to direct-bill Medicare or Medicaid. Please print out our Insurance Billing Authorization. Once you print and complete the form, it has to be mailed or faxed to us. You can mail the form to: 2001 Independence, Cape Girardeau, MO 63703 or fax to: 573-334-0493.

Finally, once we have your information all you have to do is call us toll-free at:
