Medicare or Medicaid If you're on Medicare or Medicaid, we need to know some information about you before we can process your first order. Please take a moment and fill out our Request For Patient Information. Then, we need your written approval to direct-bill Medicare or Medicaid. Please print out our Insurance Billing Authorization and follow the directions at the top of the page.
